I understand that dance training can place stress on the body and carry with it the risk of injury. I assume full responsibility for personal injury and agree that staibdance, Inc., Liceo Scientifico G. Salvemini, or any related parties shall not be held liable in any way for injuries sustained during attendance in any staibdance summer intensive classes or activities.

I give staibdance the right and permission to use, reuse, publish, republish, post, and display any photographic pictures and/or video taken, or reproductions thereof, in any medium now or hereafter known, for any legitimate purpose whatsoever.
